The annual licensing fee for a North Dakota Nursing Home Administrator License is $150.00 and must accompany the renewal application linked below.

Please complete all required pages of the renewal form, sign, and return it to the Board of Examiners office no later than December 31 of the current year. See Late Renewal information.

Please note: This document is a fillable pdf form. You can complete it online but you will also need to print it off and mail it in with your payment.

In order to assure and maintain competence in the field of nursing home administration, the Board of Examiners requires licensed administrators to attend approved classes, seminars, or conferences; the minimum time required is 20 hours per year.

    Administrators. Verification required.
College credits relating to management, finance, human resources, or gerontology:
     Each Quarter credit hour will equate to 9 clock hours
     Each Semester credit hour will equate to 12 clock hours

Please be advised that governing boards of nursing facilities should be aware of the licensing rules and regulations as they pertain to having a licensed long term care administrator managing a licensed long term care facility. Any facility that does not currently have a licensed long term care administrator is out of compliance with state law and the ND State Health Department will be notified.

Late Renewal Policy

ACTIVE AND INACTIVE LICENSE RENEWALS ARE TO BE RECEIVED BY DECEMBER 31 OF CURRENT YEAR

RENEWALS RECEIVED WITH A POSTMARK AFTER DECEMBER 31 OF CURRENT YEAR WILL BE ASSESSED A $25 PER MONTH LATE CHARGE. IF A RENEWAL IS NOT RECEIVED BY JUNE 30 OF THE YEAR FOLLOWING THE RENEWAL YEAR, THE LICENSE CAN NOT BE RENEWED. NOTIFICATION WILL BE SENT TO THE STATE HEALTH DEPARTMENT INDICATING ACTIVE LICENSEE’S WHO HAVE NOT RENEWED BY JANUARY 1.
